You will not be able to change back to your former UnitedHealthcare Plan, once you elect Harmony HMO. You will need to complete the Enrollment Form and change to a Harmony HMO Network provider. If your primary care provider is not part of the Harmony HMO Network, you may choose to elect SignatureValue Harmony HMO. Contact the Administrative Trust Funds Office if you have any questions. You may continue to use your pharmacy benefits as usual. There is no change to your pharmacy access/benefits. You will continue to have the same plan benefits.
